I would like to take this opportunity to thank you for choosing Virtually Accountable for all your Plan Management and/or Support Coordination needs (there is a flyer included in the link below to explain what the difference between these services are). We are thrilled to have you with us.
Included in your registration was a service agreement which explains what services we provide, and the terms and conditions, consent to share information with providers around your services and invoices, and information on feedback and complaints.

For Plan Management services, please advise all your providers to send their invoices to: firstname.lastname@example.org
You will receive monthly spending updates. There is also an option for the app careview to monitor your budget. Please let us know if this is required. If you wish to pre-approve your invoices, please have providers email them to you directly and then forward to Virtually Accountable when you are happy with them.

Please note in the service agreement an option for automatic renewal to elect “I would like to continue services indefinitely with Virtually Accountable and will indicate with notice should I wish to cease services. (This means my agreement will renew automatically with a new plan in the future unless I advise in writing otherwise.)”
For Support Coordination this will need to be renewed when your plan is reviewed. Please refer to flyer included “What to Happens Now?
